Sea urchin stings and spines
Another sea urchin foot before lunch on a Tuesday, this one a snorkeler who found the rocks at Fort Zachary Taylor the hard way and hopped in wincing, sole full of black spines. Here is the answer before the biology. Soak the foot in hot water for the pain, roughly 110 to 113°F, as hot as you can comfortably stand. Pull only the spines you can actually grip with tweezers. Then stop digging, because the excavating does more damage than most of the spines ever would. Wash it, check your tetanus status, and watch it for a few days. That is the whole first hour, and it handles the large majority of urchin encounters on this island.
What just happened
You met a long-spined sea urchin, and its defense is exactly what it sounds like: brittle spines of calcium carbonate that snap off in skin and crumble the moment you squeeze them, which is why grabbing at them with fingers accomplishes nothing but frustration. Two things surprise people. First, the puncture often stings and burns out of proportion to its size, because there is mild venom along for the ride. Second, the alarming purple or black stain that blooms under the skin is frequently just spine dye tattooing you, not a retained fragment. The dye fades over days on its own. It looks worse than it reads.
The first hour
Hot water is the workhorse. Soak the area at about 110 to 113°F for 30 to 90 minutes; the Wilderness Medical Society's practice guidelines on marine envenomation put hot water immersion first for pain relief, on the same heat-labile-venom logic that runs the jellyfish column. While it soaks, remove any spine that presents a graspable end with clean tweezers, gently, no excavation. Afterward wash with soap and fresh water the way you would any reef wound. Then check your tetanus status honestly: a dirty marine puncture is precisely the wound tetanus boosters exist for, and "probably fine" is not a vaccination record.
Spine removal realities
Here is the part that fights common sense. Fragments left deep in the foot mostly dissolve or get walled off by the body over the following weeks, and chasing them with a needle at the kitchen table trades a tiny foreign body for a bigger wound and a real infection risk. Leave the deep ones alone. Some do form a granuloma, a small firm lump, weeks later, and that is a clinic visit for a clean removal, not a bathroom project. The rule that keeps people out of trouble: grip and lift what you can see and reach, and refer the rest to time or a physician.
When a puncture is not simple
Most urchin punctures are shallow and boring. The ones that are not tend to sit over a joint or a tendon, where a spine or its bacteria can seed deep inflammation or infection that a foot is not built to shrug off. The warning pattern is pain that improves and then returns, new stiffness, or a joint that will not move through its normal range. That combination earns imaging and an in-person exam, because a retained fragment near a joint is a different animal from one buried in the fat pad of the heel.
Respect the animal, and the reef
Worth a paragraph, because it changes how you move in the water. Long-spined urchins graze the algae that would otherwise smother coral, which makes them unlikely heroes of reef health, and the Caribbean has twice watched them vanish: a mass die-off in 1983 and 1984 that erased about 98 percent of them, and a second one in 2022 that USGS and partners traced to a scuticociliate parasite. Every survivor matters more because of that history. So wear boots on the rocky entries, shuffle and watch your feet wading the flats and around dock pilings, and touch nothing. Do I need every spine out?
No. Pull the ones with a graspable end, and leave the deep fragments alone; most dissolve or get encapsulated by the body. Digging for them causes more harm than they do. The occasional late granuloma is a simple clinic removal.
How hot should the soak water be?
About 110 to 113°F, hot but comfortably tolerable, for 30 to 90 minutes. Test it on an unstung patch first, and never scald; the goal is steady heat that outlasts the pain, not a burn stacked on a puncture.
The spot turned purple. Is a spine still in there?
Often not. The purple is usually spine dye tattooing the skin, which fades over several days on its own. A retained spine tends to announce itself with focal pain or a firm lump rather than a flat stain.
Do I need a tetanus shot?
Check your status. A marine puncture is exactly the kind of dirty wound tetanus boosters are designed for, so if you are not current, or cannot remember, this is a good reason to get updated promptly.
